Enrollment Requirements

Please note, there is a 100% mandatory attendance requirement for program completion!

All students MUST be able to do/provide the following by noon, the Friday before the class start date:

Required Before Class Starts

• Picture Identification
• Social Security Card (an employer W-2 or previously filed tax return with full S.S. # listed are the only alternatives accepted)
• Completed drug screen (completed at designated lab; information sent to your email after registration fee received)
• Completed background check (must be with Careers in Care Academy selected company; information sent to your email after registration fee paid)
• Proof of state required immunizations — Tdap within 10 years, MMR (two doses), Hep B (three doses), and Varicella (two doses) or medically documented chickenpox
• Tuition paid in full
• Attend mandatory orientation (first day of class)
• Must be able to read, write, and communicate in English
• Must be 16 years of age or older. If 16 or 17 years of age, a signed agreement from a parent or guardian is required to complete enrollment.

All students MUST provide proof of the following by the final day of the classroom portion prior to clincal:

• AHA Basic Life Support CPR certification — must have AHA logo on the card (class offered at Careers In Care Academy  during regular scheduled program hours, for an additional fee)
• Single step TB skin test, clear chest X-ray, or TB blood test within one year
